How to Reduce Hospital Readmissions with Health Information Technology (HIT)

Residents in long-term care facilities are subject to frequent avoidable inpatient hospitalizations, which are costly, disruptive, disorienting, and often dangerous for frail elders and people with disabilities. To compound matters, residents are especially vulnerable to the risks that accompany hospitalizations and transitions of care, including medication errors and hospital-acquired infections. Prevention is key and necessary for resident quality of care improvements and eliminating billions of dollars in unnecessary health care expenditures placed on the Medicare and Medicaid programs. This session will take an in-depth look at the steps long-term care providers can take to reduce the rate of unnecessary hospitalizations through the use of the INTERACT II (Interventions to Reduce Acute Care Transfers) quality improvement program combined with Health Information Technology (HIT).
